[kernel-xen] Upgrade path from Xen 4.2 to 4.3.

I haven't actually tried it yet... The 4.3.1-0.1 packages are in the
testing repo now - but its currently at a 'it built and seems to work'

Testing feedback including different upgrade options is very welcome as
I only have the resources to do a handful of tests myself...
