I will soon buy something like the Siemens Gigaset C470IP - GAP compliant
DECT cordless (so it'll work with your old DECT handsets as extensions)
with a built in SIP client (via ethernet) AND normal PSTN line interface. 
By all accounts it sounds like what I have been after for ages.  You can
have two SIP calls and a PSTN call all going at the same time.

JBHifi sell them pretty cheap... $139 or something like it.
