[kernel-xen] Xen 4.1 and latest CentOS 7.2.1511

It might be possible the step 4 broke something in your installs. It 
might even be as simple as the grub configuration being messed up.

Consider purging and then reinstalling kernel-xen packages? eg...

yum remove kernel-xen*
yum clean all
grub2-set-default 0
yum install kernel-xen kernel-xen-firmware
